Star Wellness MD does not file insurance claims. All services are paid for by the patient at the time of service. We accept cash, credit card, HSA cards, and Flexible Spending Account cards.
On request, we will provide you with a superbill containing the applicable codes so that you may file for reimbursement with your commercial insurer. Superbills cannot be submitted to Medicare, and Medicare beneficiaries should not attempt to file them. We do not guarantee that any service will be reimbursed — many aesthetic and wellness services are not covered benefits, and reimbursement is determined entirely by your insurer.
All outstanding balances must be paid in full before your next office visit or before receiving supplements.
Our services are elective wellness and aesthetic services that are not covered by Medicare. If you are a Medicare beneficiary — including a Medicare Advantage plan — the services you receive here are not Medicare-covered benefits, no claim will be submitted to Medicare by us or by you, and you are personally responsible for payment in full.
If a proposed service could be a Medicare-covered benefit, we will decline to provide it and will direct you to a Medicare-enrolled provider. Medicare patients complete a short attestation as part of our consent packet.
Because we are a self-pay practice, you have the right under federal law to receive a Good Faith Estimate of what your care will cost before you receive it. We will give you a written estimate when you schedule, or any time you ask for one.

When an appointment is scheduled, that time is set aside specifically for you. We understand plans change, so we ask for 48 hours’ notice to cancel or reschedule. Requests may be submitted at any time through the patient portal, by voicemail, or in writing, and are timestamped when received. Staff are available to help by telephone Monday through Friday, 9:00 a.m. to 5:00 p.m.
If we must reschedule your appointment for clinical or emergency reasons, no fee applies and we will offer you the earliest reasonable alternative.
Most service appointments require a deposit at the time of booking, and consultation fees are collected when you schedule. Deposits and consultation fees are refundable if you cancel at least 48 hours before your appointment. With less than 48 hours’ notice, the deposit is forfeited and is not transferable to a rescheduled or different service. Consultation fees for appointments you keep are applied in full toward your treatment.
All payments for services are final. We do not offer refunds or credits for services that have been rendered.
If you are unsure whether a treatment is right for you, please tell us before you book. We would far rather spend an extra consultation talking it through than have you pay for something you did not want.
Packages must be paid in full at the time of the first service in the package.
Promotions and discounts are offered periodically and are subject to the specific terms published with each offer. Unless expressly stated otherwise, offers may not be combined with other offers, and all promotional services remain subject to consultation and medical eligibility.
Gift cards may be purchased for any of our services. Gift cards do not expire and are non-refundable.
We offer monthly memberships for certain services. A membership’s price and included benefits are published where the membership is offered and confirmed when you enroll. All memberships are billed automatically each month to your card on file, and all of them follow the same rules:
If a renewal charge fails, we’ll contact you before treating it as a past-due balance under this policy.
Your provider may recommend blood, saliva, stool, urine, hair, or skin testing within their scope of practice. Some tests are ordered through specialized laboratories to assess structural or functional deficiencies; these are not always diagnostic on their own, but can provide information that meaningfully improves your care. You will always have the opportunity to discuss a test’s usefulness and limitations with your provider before any sample is collected.
Laboratory fees are separate from your visit fee. You agree to pay the laboratory any fees due for sample collection and processing. If you choose to use insurance benefits for laboratory testing, that billing relationship is between you, your insurer, and the laboratory. Self-pay lab pricing is available where applicable.

We keep a credit card securely on file for each patient. By signing our consent and policy form, you authorize Star Wellness MD to charge that card for any balance that remains outstanding more than 15 days, and for any charges you have authorized.
If your card on file is charged, we will send you a receipt showing what was paid within 30 days. This program does not affect your ability to dispute a charge. Please keep us updated if your card information changes.
If an account becomes past due beyond 15 days, the credit card on file will be charged. If that card declines and we are unable to resolve the balance with you, the account may be referred to a collection agency, and the cost of collection may be added to your balance. Unpaid accounts referred to collections may be reported to credit bureaus. If collection must be referred to an attorney, you agree to pay reasonable attorney’s fees and court costs.

Many supplements, vitamins, medical-grade foods, nutritional powders, botanicals, and homeopathic remedies have not been evaluated by the U.S. Food and Drug Administration. These products are not intended to diagnose, treat, cure, or prevent any disease.
No refunds, credits, or exchanges are available on supplements of any kind. Once these items have been purchased or have left the office, they cannot be returned. Special orders must be paid for at the time of order and cannot be refunded, exchanged, or modified once placed.
Supplements are not included in your visit fee. They may be purchased through our online dispensary or from any dispensary you prefer. Please tell your provider if you are vegetarian and need vegetarian formulations.
Star Wellness MD provides specialized, service-specific medical care — aesthetics, weight loss, hormone therapy, and wellness treatments. We are not a primary care practice. We do not provide comprehensive medical management, routine screenings, or urgent care.
We reserve the right to decline service at any time, to anyone, for any lawful reason — including where treatment would not be medically appropriate.

Star Wellness MD may update this financial policy at any time. The current version is always available at our office and on this page, and the “last updated” date above will change when it is revised. Terms that affect money you have already paid — a package purchased, a deposit made — are governed by the policy in effect when you paid.
