Good Faith Estimate notice · Star Wellness MD, PLLC
Last updated August 5, 2026
Under federal law — the No Surprises Act — people who are uninsured, or who have insurance but choose not to use it, have the right to receive a written estimate of the expected cost of their care before they receive it. That document is called a Good Faith Estimate.
Because Star Wellness MD is a cash-pay practice and does not bill insurance, this right applies to essentially every patient we see. Honest, upfront pricing is how we prefer to practice anyway — this is simply that commitment, written down and required by law.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency care. Ask us for one at any time, or before you schedule. If you receive a bill that is at least $400 more than your estimate, you can dispute it.
Your Good Faith Estimate lists the expected charges for the care you are scheduling — the visit itself, and the items and services reasonably expected to go with it, such as medications administered in the office, injectables, or in-house procedures.
Some costs are billed by someone other than us and will not appear in our estimate:
An estimate reflects the plan as it is known when it is written. Your care may change as your provider learns more — a dose adjusted, a treatment added or removed. If your plan changes materially, ask us for an updated estimate, and we will provide one.
| You schedule care at least 10 business days ahead | Within 3 business days of scheduling |
| You schedule care 3–9 business days ahead | Within 1 business day of scheduling |
| You request an estimate without scheduling | Within 3 business days of your request |
You will receive it in writing — on paper or electronically, whichever you prefer — and we will keep a copy in your record. You are also welcome to ask questions about it before you decide anything. Nobody at this practice will rush you into care you have not had a chance to price out.
If you receive a bill that is at least $400 more than your Good Faith Estimate for the same care, you may dispute the bill through the federal patient-provider dispute resolution process.
Please call us first. If a charge does not match what you expected, call the office at (210) 775-1785. Most differences are a misunderstanding we can resolve in one conversation — and you keep every right described here regardless of how that conversation goes.
For questions about your rights, more information about the dispute process, or to start a dispute, visit cms.gov/nosurprises or call the federal help line at 1-800-985-3059.
Just ask — when you book, at your consultation, or any time before you decide.

Our published rates for most services are on the pricing page, and payment terms are in our Financial Policy. A Good Faith Estimate goes further: it prices your specific plan of care, in writing, before you commit to it.
