How It Works
01
Your symptoms, medical and menstrual history, prior hormone use — and education on what’s changing and every treatment option, from creams to pellets.
02
A full baseline hormone panel — use your medical insurance, or $200 self-pay through us.
03
Full lab review, your diagnosis explained, a custom treatment plan, prescriptions or pellet ordering, and informed consent — risks, benefits, and alternatives.
04
Labs and dose adjustment at 6 weeks, symptom and lab follow-up at 3 months, then maintenance every 3–6 months.
Estradiol · Progesterone · Testosterone (Total & Free) · Dehydroepiandrosterone Sulfate (DHEA‑S) · Follicle-Stimulating Hormone (FSH) · Luteinizing Hormone (LH) · Sex Hormone‑Binding Globulin (SHBG) · Full Thyroid Panel · Vitamin D · Hemoglobin A1c (A1c) or Fasting Glucose · Complete Blood Count (CBC) · Comprehensive Metabolic Panel (CMP) · Lipid Panel — and others as needed — a comprehensive baseline for perimenopause or menopause, not a two-line screen.
Treatment Options
Whether you’re navigating perimenopause or fully in menopause, your comfort, metabolism, and lifestyle decide the delivery method — pills, patches, creams, vaginal therapy, or long-acting pellets.
The foundation
For hot flashes, night sweats, mood, vaginal dryness, and bone health. Options include daily oral estradiol, weekly patches, topical gels and sprays, vaginal estrogen for dryness and painful intimacy, and long-acting pellets.
Balance & sleep
Balances estrogen, protects the uterine lining, and can improve sleep and mood. Nightly oral micronized progesterone, topical cream, or pellet options.
For women with a uterus
When estrogen is used with an intact uterus, progesterone is combined to protect the endometrium — via combination tablets, patches, or pellets with both hormones.
Libido & energy
Sometimes prescribed off-label for women to improve libido, energy, and sexual satisfaction — as a specially formulated cream or long-acting pellet, always physician-dosed and monitored.
Intimate comfort
FDA-approved vaginal DHEA (Intrarosa®) for dryness and painful intimacy, plus additional options like ospemifene where appropriate.
When HRT isn’t an option
Hot flashes can also be treated without hormones — including SSRI/SNRI, gabapentin, and clonidine approaches — so there is a path for every woman, including those who can’t take HRT.
Pellet Therapy Spotlight
Pellets are the delivery method our patients love most: small implants placed under the skin (usually the hip) in a quick in-office visit — then months of steady hormone release with no daily dosing. That steadiness matters most in perimenopause, when hormones swing rather than decline, and it’s just as valuable once you’re fully in menopause — fewer of the fluctuations that make you feel like you’re on a rollercoaster, either way.
Pellet Insertion
Steady hormone levels for 3–4 months — no daily routine
Initial pellet included · additional pellets $75 each · performed by a physician
Honest Medicine
Hormone therapy is powerful medicine, and we’ll always tell you the whole truth about it.
Breast, ovarian, or uterine cancer · blood clots or stroke · uncontrolled high blood pressure · liver disease. If that’s you, our non-hormonal options can still help.
HRT requires regular lab monitoring and up-to-date mammograms and Pap smears — we’ll keep you on schedule. Possible side effects like bloating, breast tenderness, acne, or mood changes are usually dose-dependent and manageable with adjustment.
Common Questions
Perimenopause isn’t menopause — it’s the years leading up to it, often starting in your late 30s or 40s, while your cycle is still happening. Because periods haven’t stopped, many doctors rule out hormones entirely, which is exactly how so many women end up hearing “your labs are normal” while feeling anything but.
The reason perimenopause feels so confusing is that hormones during this phase don’t decline steadily — they fluctuate, sometimes wildly, month to month. A normal cycle one month and a miserable one the next isn’t inconsistency on your part; it’s the pattern itself. That instability is often what makes women feel like something’s wrong with them, when it’s actually just biology in transition.
If any of this sounds familiar and your cycle hasn’t stopped, you’re not too young for this conversation, and you’re not imagining it. The evaluation and treatment options on this page — labs, estrogen and progesterone therapy, pellets, and everything else — apply just as much to perimenopause as to full menopause; timing your labs to your cycle simply lets us read the fluctuation more accurately.
Fatigue, brain fog, mood swings, anxiety, hot flashes, night sweats, sleep disturbances, weight gain, low libido, and vaginal dryness are common signs of shifting hormones in perimenopause and menopause. A physician evaluation with a comprehensive hormone panel can tell you what’s actually happening — even when routine labs have been called “normal.”
The initial hormone consultation is $150. Comprehensive labs run through your medical insurance, or $200 self-pay. The program start and lab review visit is $99, and ongoing follow-up visits are $120 each — typically at 6 weeks, 3 months, and every 3–6 months thereafter. No memberships required, no surprises.
Pellets are small hormone implants placed under the skin (usually the hip area) in a quick in-office procedure. They release steady hormone levels for about 3–4 months, reducing fluctuations and removing daily dosing. Insertion with your initial pellet is $299; additional pellets are $75 each.
For many women, modern hormone therapy is safe and effective when individualized and monitored. It’s not recommended with a history of breast, ovarian, or uterine cancer, blood clots, stroke, uncontrolled high blood pressure, or liver disease. Therapy requires regular lab monitoring and up-to-date mammograms and Pap smears — and if HRT isn’t an option for you, non-hormonal treatments are available.
