These Terms & Conditions (“Terms”) govern your use of the website located at starwellnessmd.com and any related pages, forms, booking tools, and communications operated by Star Wellness MD, PLLC (“Star Wellness MD,” “we,” “us,” or “our”), a Texas professional limited liability company located at 1652 Pat Booker Road, Universal City, Texas 78148.
By using this website, submitting a form, booking an appointment, or communicating with us electronically, you agree to these Terms. If you do not agree, please do not use the website.
These Terms govern your use of the website. The care you receive as a patient is separately governed by the consents, treatment agreements, and financial policies you sign at the practice, and by our Privacy Policy and Notice of Privacy Practices. Where a signed document you receive as a patient conflicts with these Terms, the signed document controls for that care.
The content on this site — service pages, blog posts, symptom checks, pricing information, and educational material — is provided for general information only. It is not medical advice, and it is not a substitute for evaluation, diagnosis, or treatment by a qualified healthcare professional who knows your history.
Using this website does not create a physician-patient relationship. That relationship begins only when you are evaluated by one of our providers and accepted for care. Submitting a form, taking our hormone symptom check, requesting an appointment, or emailing us does not, by itself, establish care or obligate us to treat you.
Our symptom check and similar tools are educational self-assessments. They do not diagnose any condition, and their results are not a treatment recommendation.
In an emergency, call 911 or go to your nearest emergency department. Do not use this website, email, text messages, or the patient portal to report a medical emergency or an urgent change in your condition. These channels are not monitored continuously and messages may not be seen for some time.
This website is intended for adults 18 years of age or older. By using it, you represent that you are at least 18 and legally able to enter into these Terms.
Any patient under 18 who is treated at our practice must be accompanied by a parent or legal guardian who provides written consent. Certain services are not offered to minors at all. We do not knowingly collect personal information online from anyone under 13.
We provide services in the State of Texas. Telehealth visits are available only to patients physically located in Texas at the time of the visit.
We reserve the right to decline to treat any individual where, in our clinical judgment, treatment would be unsafe, medically inappropriate, or outside our scope of practice — and to end a physician-patient relationship consistent with Texas law and medical ethics.
Individual results vary. Every description of a treatment, timeline, or outcome on this site describes what is typical or possible — not what you are guaranteed. Response to medical weight loss, hormone therapy, hair restoration, PRP, neurotoxins, fillers, and IV or IM therapy depends on your physiology, health history, adherence, and factors outside anyone’s control. No specific result is promised.
Whether a treatment is appropriate for you is a medical decision made at your consultation. We may recommend a different treatment than the one you inquired about, a smaller amount of treatment than you requested, or no treatment at all. Publishing a service or price on this website is not an offer to provide that service to any particular person.
Some treatments we offer involve compounded medications or products used off-label. Where that applies, it is disclosed to you before treatment, in writing, as part of your consent. Compounded medications are not FDA-approved products, and their preparation is governed by the compounding pharmacy’s licensure.
Prescriptions are issued at the sole discretion of the treating provider following an appropriate evaluation. We do not accept prescription requests, refill requests for controlled substances, or dosage-change requests through this website, social media, or text message.
Appointments may be requested online through our scheduling system or by telephone. A requested time is not confirmed until you receive confirmation from us. Online scheduling is provided through a third-party platform; your use of that platform is also subject to its own terms and privacy practices.
Consultation fees, deposits, cancellation windows, no-show charges, refunds, package expiration, and membership billing are governed by our Financial Policy, which is incorporated into these Terms by reference and provided to you as a patient. That policy — not this page — is where fees, notice windows, and refund terms are stated, and published rates live on our pricing page.
We may need to reschedule your appointment for clinical, staffing, or emergency reasons. We will offer you the earliest reasonable alternative.
Star Wellness MD is a cash-pay practice. We do not bill insurance, Medicare, or Medicaid for our services, and we do not submit claims on your behalf. Payment is due at the time of service. Superbills, Medicare rules, laboratory billing, and financing are covered in our Financial Policy.
Prices for products and services are subject to change without notice. Typographical or display errors in pricing on this website do not obligate us to honor an incorrect price; we will tell you the correct price before you are charged.
Promotional offers published on this website are valid only for the dates stated in the offer and only while appointments remain available. Unless the offer says otherwise:
We reserve the right to modify, suspend, or end any promotion at any time, and to limit quantities or the number of redemptions per patient.
We offer telehealth visits to patients physically located in Texas. By participating, you acknowledge that:
You are responsible for being in a private, safe location for your visit, and for providing your current physical location so that emergency services can be directed to you if needed.
Telephone consultations (voice only, no video) may be offered where appropriate. They carry additional limitations — no physical examination and no visual assessment — and your provider may determine during the call that safe care requires a video visit or an in-person appointment instead.
When you provide your phone number or email address, you consent to receive appointment-related communications from us, including confirmations, reminders, results notifications, and care instructions.
Marketing messages are separate. We send promotional emails and text messages only to people who have opted in to receive them. You may opt out at any time — reply STOP to any marketing text message, or use the unsubscribe link in any marketing email. Opting out of marketing does not stop appointment-related messages necessary for your care. Message and data rates may apply, and message frequency varies.
Please do not send detailed health information by email or text. These channels are not fully secure. Use the patient portal for clinical questions, records, and anything involving your medical history. How these channels are protected, what becomes part of your record, and which topics should never go by email or text are covered in our Privacy Policy.
Do not use email, text, social media, or website forms for urgent needs. If you have not received a reply within one business day, please call us at (210) 775-1785.
The patient portal is provided by a third-party vendor. You are responsible for keeping your login credentials confidential and for all activity under your account. Tell us immediately if you believe your account has been accessed by someone else.
Clinical photographs may be taken as part of your care to document baseline appearance and track progress. These are part of your medical record. We will never use your photographs in marketing, social media, or on this website without your separate, specific, written authorization, which you may revoke at any time going forward.
Before-and-after images shown on this website or our social media are of actual patients who authorized their use, unless expressly labeled otherwise. They are not retouched to alter results. They illustrate one person’s outcome and are not a prediction or guarantee of yours.
Testimonials reflect the individual experience of the person giving them. They are not offered as typical results, and no one is compensated for a testimonial unless that relationship is disclosed alongside it.
If you post a public review or comment about us, please know that privacy law limits how we may respond: we cannot confirm or deny that any person is a patient, or discuss care, in a public forum. A brief, generic reply inviting you to contact us directly is not an acknowledgment that you received care here.
If you submit content to us — a review, comment, photo, or message — you grant us a non-exclusive, royalty-free license to use it for the purpose for which you submitted it. You represent that the content is yours to share and does not violate anyone else’s rights.
All content on this website — text, layout, photography, graphics, logos, the Star Wellness MD name and marks, the STAR Method™ framework, patient education materials, and the design of these pages — is owned by Star Wellness MD, PLLC or used with permission, and is protected by copyright, trademark, and other laws.
You may view, print, and share our patient education content for your own personal, non-commercial use. You may not copy, republish, scrape, adapt, or use our content, photographs, or branding for commercial purposes, including for a competing practice, without our written permission.
You agree not to: interfere with or disrupt the website; attempt to gain unauthorized access to any system or account; use automated tools to harvest content or contact information; submit false information or impersonate another person; upload malicious code; or use the website in violation of any applicable law.
Third-party names and product marks referenced on this site — such as Botox®, Xeomin®, Kybella®, Juvéderm®, Cherry, and CareCredit — belong to their respective owners. Their use here is descriptive and does not imply endorsement or affiliation.

Important: nothing in these Terms limits, waives, or releases any liability for professional medical negligence, or any other liability that cannot be limited under Texas law. Your rights as a patient are not altered by using this website.
You agree to indemnify and hold harmless Star Wellness MD, PLLC and its owners, providers, and staff from claims, damages, and reasonable costs arising out of your misuse of this website, your violation of these Terms, your infringement of another person’s rights, or content you submit to us. This does not apply to claims relating to the professional care we provide.
These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-laws rules. Any dispute relating to this website or these Terms will be brought exclusively in the state or federal courts located in Bexar County, Texas, and you consent to that jurisdiction and venue.
Before filing any claim, we ask that you contact us first and give us a reasonable opportunity — at least thirty (30) days — to resolve the matter directly. Most concerns are resolved with a conversation.
Claims relating to medical care are governed by Texas law applicable to healthcare liability claims, including any applicable notice requirements, and by any separate agreement you signed as a patient.
We may update these Terms as our services, technology, or the law change. The revised version takes effect when posted, and the “last updated” date at the top of this page will change. Your continued use of the website after that date means you accept the revision. Material changes affecting your care or your financial obligations will be communicated to you directly as a patient, not only by posting here.
If any provision of these Terms is found unenforceable, the rest remains in effect. Our failure to enforce a provision is not a waiver of it.
